Procedure abbreviations

BMI: Select the correct range of your patient's BMI.  These correspond to World Health Organization BMI Classifications of
Underweight         <18.5
Normal                    18.5-25
Overweight             25-30
Class I Obese          30-35
Class II Obese        35-40
Class III Obese       >40


Age: Enter the patient's age

Fully independent: Check no if patient has required assistance with some or all of ADLs in the past 30 days.  

Smoker: Check yes if the patient has been a smoker within the past year

Dyspnea: Check yes if patient has dyspnea with exertion or at rest

Steroid or immunosuppressant use: Check yes if the patient has been treated with systemic steroids or other immunosuppressants (TNF inhibitors etc.) in the past 30 days

History of Stroke: Check yes if the patient has ever had a stroke, not including a transient ischemic attack (symptoms persisted <24 hours)

Disseminated cancer: Check yes if the patient has widely metastatic solid or hematologic malignancy

Weight loss:  If the patient has unintentionally lost >10% of their total body weight in the past 6 months, check yes

Transfusion: Check yes if the patient has more than 4 units of red cells transfused in the 72 hours prior to surgery

Radiation treatment:  Check yes if the patient has received radiation treatment in 90 days prior to surgery

Preoperative sepsis: Check yes if the patient meets criteria for having sepsis at the time of surgery

Preoperative open wound: Check yes if the patient has an open wound (with or without infection) at the time of surgery

INR checked:  Check yes if the patient has had an INR checked in the 30 days prior to surgery for any reason

White count: Enter the patient's most recent preoperative leukocyte count

Hematocrit: Enter the patient's most recent preoperative hematocrit
